No fall protection. There should be a fall protection system in place for workers on the roof, such as a fall restraint system to prevent workers from falling to a lower level.

Ladders should be at least 3 feet above the roof access and secured (tied off)to prevent movement.  

All of the workers should be provided correct PPE including eye protection, hard hats, gloves, etc.
